Your Jamboree Medical Forms will come to you via e-mail. It will be
encoded with your specific information. Do Not copy it or use
someone else's form. All Winnebago Council Jamboree scouts and their
leaders will need to have their Jamboree medical forms filled out by
January 31, 2010. If you are an adult
or youth staffer, you will send your forms into National BSA.
Please fill in all lines on the medical form. Do not leave anything blank. Medical forms will be reviewed by the Winnebago Council Jamboree committee, the Council office and then be forwarded on to the National office, who may also review them.
The following memo came from National BSA Jamboree office. Once you have signed up for Jamboree on-line, you will likely receive this memo. Watch for your medical forms on-line.
Excitement builds as we get closer to the 2010 Jamboree and so does the need for more preparation. The time to submit your Annual Health and Medical Record is nearing. Please read the attached carefully regarding the medical process for the 2010 Jamboree.
